Learning Disability Intensive Support Services
The team consists includes Speech and Language Therapy, Occupational Therapy, Psychology, Seconded Positive Behaviour Practitioners from the Council, a seconded Provider Representative, Positive Behavioural Management Lead Practitioner, Support Workers and Qualified Practitioners (made up of a Health and Exercise Practitioner, Learning Disability and Mental Health Nurses).
The service objectives are:
- To aim for the best possible outcomes for individuals
- To help people to live in their homes and to stay at home
- To use a Positive Behaviour Support approach
- To provide practical support to families and people who are important to the service user
- To work in a multi-disciplinary way
- To support the individual and provide practical support and guidance to the people who work with them
- To provide training, advice and guidance in the community to providers and carers
- To provide support to adults and children
The service provides intensive interventions to support service users at home across a seven day period up to 24 hours per day dependent on individual need. The service is available 365 days a year and can provide 24 hour telephone advice and support to people open to its service that are in crisis. Should people require crisis support from LDISS a planned visit will be booked for the following working day.
The LDISS is available to all Gloucestershire residents who are Gloucestershire GP registered or funded by NHS Gloucestershire / Gloucestershire County Council, who also have a diagnosis of Learning Disability and complex health needs that cannot be met by mainstream or other specialist services alone.
In order to be truly preventative, the service also supports people in the transition from child to adult services (14-18 yrs) with agreement from the lead commissioner. Referrals are able to be made to Commissioners, Children and Young Persons Service (LD), Community Learning Disabilities Teams and the Learning Disability Inpatient Service.
The service provides training in Positive Behaviour Support, and reactive strategies in line as part of unifying approaches within the County. Ongoing evaluation of LDISS is facilitated through the Commissioning Strategy Group, and LDISS reports to the Challenging Behaviour Working Group.
