Intensive Health Outreach Team

The Intensive Health Outreach Team (IHOT) provides assessment and treatment for adults with profound and multiple learning difficulties, and other vulnerable adults with associated health problems. 
​The IHOT team is made up of a group of dedicated and skilled practitioners, committed to providing a person-centred service in a variety of settings including: hospital, 24 hour staffed supported accommodation and the service user’s home, recognising how a care pathway relates to each individual and provides individualised care with the collaboration of the right health partners achieving equitable health outcomes.

The service is primarily community based. The service supports the most vulnerable people with a learning disability. This includes people with profound and multiple disabilities and older people with a learning disability whose health needs require a level of intensity and specialist intervention not currently provided by the Community Learning Disability Team (CLDT).
 
To be eligible for referral to the IHOT team you must be open to the CLDT or be known to the CCG. The individual must be aged over the age of 18 and have a diagnosis of a learning disability (impaired intelligence and social functioning with a lasting effect on development) with profound and multiple or complex health needs issues associated with their learning disabilities, or an older person with a learning disability whose health needs require a level of intensity and specialist intervention not provided by the CLDT.
 
The service is considered unsuitable for:
• those persons who do not have a learning disability
• when intervention is within the capacity of the CLDT
• when the referral issue is exclusively one of social care, where health needs are not associated to learning disability.
 
If the referral is considered unsuitable, the IHOT team will signpost to relevant known statutory and non-statutory agencies and inform the care coordinator with a rationale for the decision. The team follows a person-centred approach, each referral will be considered on its own merits. The team acknowledge that service users do not always fit written definitions. 
 
The IHOT team will offer 365 days nursing availability per year with flexibility during any 24 hour period dependent on client need. Handovers entail a review of all individuals on the team’s caseload, any recently received/emergency referrals will be triaged and actioned as necessary.
